The Transition Program

The Transition Program will include four weeks of assessment and skill development followed by a gradual placement into a supported work environment.  Staff will work with youth as well as employers to ensure placements are a good fit and expectations are clearly outlined. Youth will work twenty-four hours/week with employers and reconvene as a group at PLEC for six hours/week to continue their group based learning and address any issues they may be facing.

Youth in the Transition Program will achieve the following outcomes:

  1. Identify existing skills and competencies;
  2. Identify career interests;
  3. Explore one or more careers;
  4. Develop teamwork skills;
  5. Practice speaking in public;
  6. Explore and practice the elements of a successful job interview;
  7. Learn about different educational opportunities;
  8. Create a portfolio;
  9. Earn certificates specific to their employment goals such as First Aid and CPR, WHIMS, Food Handlers Education Course, Super Host, etc.;
  10. Learn and practice how to set goals;
  11. Develop an action plan;
  12. Participate in a work placement;
  13. Develop interpersonal and self awareness skills;
  14. Understand employer expectations;
  15. Develop problem solving and conflict skills; and
  16. Practice skills and receive feedback. 

Using an adult education model, the Learning Facilitator will design and deliver life skills, literacy, numeracy, pre-employment and employability curricula based on an assessment of the learning needs of youth participants.  The Learning Facilitator will help youth develop a plan that includes skill development, work placements and future goals.  The Learning Facilitator will assess clients’ needs as they relate to pre-employment, life-skills, academic and employment success and work collaboratively with other Phoenix staff and community partners to ensure plans are well coordinated.  Each year, this will be a 10-month position from September to June.
