Follow Up Program

"Not everyone who accesses the Centre or its other programs has the same needs. That is why the staff pay close attention to each individual and how their needs can be met. One of the important things to me was that everyone has a name. No code or ID number. Not Sir, Mister, Madam or Doctor. This makes you feel at home and not in an institution. Before I found Phoenix, I had no home... no name... no food... no clean clothes... nowhere safe to go. Since being part of the Follow Up Program, I have a safe place, food to eat, clean clothes, a wonderful partner, a three-year-old daughter and a job. I'm living clean and sober and I am free."

Staying Connected

The Follow Up Program is an “after care” program, which provides ongoing support to youth who have lived in Phoenix House or the Supervised Apartment Program. Established in 1988, the Follow Up Program provides the final step in the Phoenix continuum of care. Recognizing that young people need ongoing supportive relationships as they move toward independence, the Follow Up Program seeks to maintain relationships once clients have moved on to independent living.

Relationships are maintained through regular contact by telephone, in person, by mail and through holiday celebrations and annual summer barbeques.

Other services offered through the Follow Up Program include supportive counselling, crisis intervention, referral and advocacy services. The number of Follow Up clients increases each year as former residents move on and build families of their own. As part of our after-care philosophy, our clients' partners and children are welcomed into the program and become part of the growing Phoenix family.
